The HTA has published guidance on traceability procedures of handling the deceased to provide clarity on requirements
This is to help mortuaries mitigate risks of future incidents occurring that have an impact on the dignity of the deceased, for example, the release of the wrong body.
The guidance sets out examples of what the HTA consider good practice and examples of what does not meet HTA standards.

There is also guidance on incidents and near-miss incidents in the Post Mortem sector, specifically around ‘release of the wrong body’. It sets out the root causes the HTA have identified in incidents reported to us, as well as advice for establishments to mitigate these risks.
