Overview

To provide an understanding of the end to end death certification process post-September 2024, using a case study to illustrate the journey of 'Brian' through the end-to-end process of death certification. This lecture aims to highlight the roles of various stakeholders, the steps involved, and the impact on both the bereaved and the deceased.

About Daisy

Daisy Shale, is the Lead Medical Examiner Officer (MEO) for Wales. With a career spanning over 27 years in various sectors of the NHS, Daisy has established herself as a leader in the field of medical examiner officer and medical examiner process work.

In March 2009, Daisy took on the role of Senior Medical Examiner Officer and Pilot Project Lead at Sheffield Medical Examiner Service. Since December 2019, Daisy has been the Lead Medical Examiner Officer for Wales. Daisy also serves as a member of the Royal College of Pathologists Medical Examiners’ Committee, representing the interests of medical examiner officers and is also the course director for Medical Examiner Officer training at the Royal College of Pathologists and has authored two e-learning modules and a chapter in "The Medical Examiner Service: A Practical Guide for England and Wales." She has also trained over 1500 medical examiners in England and Wales.

Daisy is an active member of several steering groups, including the Welsh Bereavement Steering Group and the All Wales Mortality Steering Group. She has played a key role in producing guidance on improving bereavement services and incorporating healthcare quality standards into mortality review and patient safety improvements.
